Social Support Service

The Social Support Service carries much varied information. This includes information on rest homes and community hospitals, and can refer for assessment or, in some cases, assess for placement. It can also tell about benefits, and can assist with applying for these.

It also covers discharge planning and helps family/whanau to look at the best ways of utilizing help available.
The Social Support Service is an essential part of the care provided by the Clinical Team.

Social Work

The Social Support programme at North Haven Hospice provides a co-ordinated approach to accessing patient care, and aims to ensure that the patient and family/whanau receive the maximum benefit from services available in the community.

This includes advice on and referral for assistance available in the home via community agencies and continuing contact with the agencies providing this care.
